Choose Action Wellness for your primary care needs! Our Annex West Health Center can provide you and your children (ages 12 and up) with regular checkups and sick appointments as well as free HIV and STI testing. Walk-ins are welcome, but appointments preferred. To make an appointment, please call 267-350-5150.

  • Mondays 8 am to 4 pm
  • Tuesdays: 8 am to 4 pm
  • Wednesdays: 9:40 am to 5 pm (Suboxone treatment available. Please call Mariam Malik at 267-864-1261.)
  • Thursdays: 8 am to 4 pm

Who We Are

In January, 2015 ActionAIDS opened Annex West clinic (our first primary care clinic), in collaboration with The Family Practice and Counseling Network (FPCN), a program of Resources for Human Development. Annex West was created based on the medical home model.

Clinical Staff

Clinical Staff

The clinic is staffed by FPCN nurse practitioners and integrated with medical case management in order to:

(a) ensure that people living with chronic diseases receive a continuum of care according to their specific needs, and

(b) maximize retention and adherence to care.

A behavioral health specialist is available on site during clinic hours to meet with patients to provide immediate support with mental health and addiction issues. 

Those patients with ongoing needs are linked to services by our medical case managers.  Patients are also able to easily access Action Wellness’ vocational specialist for employment assistance and housing counselors for access to housing resources.
