Action Wellness’ housing programs offer both transitional and permanent housing assistance. Medical Case Managers and Housing Counselors work with program participants to support housing stability, adherence to medical care and improved health outcomes.

Arms Around You

Arms Around You is a transitional housing and supportive services program. Each client is assigned a Housing Counselor who assists with housing searches, budget counseling and linkage to utility assistance programs. Medical Case Managers assist with linkage to medical care, medication adherence, transportation and public benefits. Our team will work together with program participants to develop care plans, with the goals of medical care adherence and locating permanent, sustainable housing. The program will pay a portion of a program participant’s monthly rent for up to twenty-four months or until permanent housing is located. All units are inspected prior to lease signing and annually to ensure that participants are placed in safe, affordable housing.

Eligibility Criteria

  • Living with HIV
  • Currently experiencing homelessness and/or housing insecurity
  • Household income must not exceed 500% of FPL

Rapid Rehousing

Rapid Rehousing

Rapid re-housing provides rental assistance for up to one year. All referrals come from the Philadelphia Office of Homeless Services (OHS). Each program participant is assigned an Intensive Case Manager (ICM). The ICM assists clients with locating the home that best fits their housing and medical needs. Program goals include linkage to housing within one year, medical care adherence and increased self-sufficiency. Clients pay 30% of their income towards their rent and are linked to utility assistance programs, community resources and employment services to assure that they can maintain housing after the rental assistance has ended. All units are inspected prior to lease signing to ensure that participants are placed in safe, affordable housing. Programs include:

Back on Track: Single Head of Households

  • Living with HIV
  • Currently experiencing homelessness
  • Low-income household

Pathway to Home: Singles & Families

  • Living with a disability
  • Currently experiencing homelessness
  • Low-income household

Permanent Supportive Housing

Permanent Supportive Housing programs are provided to individuals who are considered chronically homeless (as per the federal definition) and/or families who are experiencing homelessness. The household must include someone living with a disability. Permanent supportive housing is provided through tenant-based rental assistance. Referrals are received from OHS. All units are inspected prior to lease signing and annually to ensure that participants are placed in safe, affordable housing.

Positive Living

Positive Living is a tenant-based rental assistance program. We provide permanent supportive housing for 18 households. Our staff offer Housing Counseling & Medical Case Management services, with the goals to support independent living and medical care adherence. Program participants also work with an Assistant Case Manager who can help with ADLs, childcare and translation services. Applicants must have HIV to apply for this program.

Project Based

Action Wellness provides project-based housing for 12 families. Our property is located in the Fairhill section in the City of Philadelphia. The apartments are two bedroom units with on-site maintenance staff. Housing Counseling & Medical Case Management services are offered with the goals to support independent living and medical care adherence. All program participants work with an Assistant Case Manager who can help with ADLs, childcare and translation services. Applicants must have HIV to apply for this program.

Housing Opportunities for Persons with AIDS

Action Wellness provides tenant-based rental for 20 households. Program participants are assigned to a Housing Specialist and Medical Case Manager. Our staff provide support with treatment goals to support independent living and medical care adherence. Participants pay 30% of their income towards rent. Applicants must have HIV to apply for this program.

Harbor Project

In partnership with Pathways to Housing, Action Wellness provides Medical Case Management services for 20 household receiving tenant-based rental assistance through the Harbor Project. Harbor Project operates from a Housing First model offering a low barrier housing approach, which quickly links individuals experiencing chronic homelessness to permanent supportive housing. Clients are linked to a full treatment team, including nurses, psychiatrists & service coordinators. The Pathways team provides supportive services to maximize housing stability and improve health outcomes.

  • A history of chronic homelessness as defined by the federal government
  • Living with HIV
  • Axis 1 Mental Health Diagnosis
